To request a hard copy of Vantage Medicare Advantage’s provider directory, please call our Member Services Department toll-free at (866) 704-0109 from 8:00 a.m.- 8:00 p.m. seven days a week from October 1, 2021 through March 31, 2022. For all other dates, Member Services may be reached from 8:00 a.m. - 8:00 p.m., Monday - Friday. TTY users should call 711. Vantage will mail a hard copy of the provider directory to you within three (3) business days of your request. Vantage may ask whether your request for a hard copy is a one-time request or if you are requesting to receive the provider directory in hard copy permanently. If you request it, your request for hard copies of the provider directory remains until you leave Vantage or request that hard copies be discontinued.
